Appointments

To schedule an appointment, please download, complete and sign the appropriate intake paperwork for the service(s) you are requesting, and return them to my office with the required deposit. You may fax the completed forms to 813.977.6105, mail them to my attention at 13067 Telecom Parkway North, Temple Terrace, FL 33637, or drop them off in person.

All fees can be paid by cash, check, or credit card. Credit card payments by phone can be made by calling my office at 813.977.4788.

When all forms and deposits are received, you will be contacted by my office to schedule an appointment at the earliest opening.
